Medical Clearances and Immunization Records

Access the Student Health Portal using your Morgan State University user name and password.

Through the portal, you will:

• Complete required health forms
• Enter immunization dates
• Upload required medical documentation
• Monitor your clearance status
• Receive secure messages from the University Health Center

Before completing your health requirements, verify that your personal information is accurate.

Please update and confirm your:

• Mobile phone number
• Morgan State University email address
• Home address
• Emergency contact information

Your profile must be complete before submitting medical documentation. This information must updated and confirmed in the portal prior to proceeding to the next step. 

Complete all required health history questionnaires within the Student Health Portal.

Depending on your individual health history, you may be asked to provide information regarding:

• Current medical conditions
• Allergies
• Prescription medications
• Previous surgeries
• Hospitalizations
• Mental health history
• Family medical history

If you report a pre-existing medical condition, indicate that you are currently receiving treatment from a healthcare provider, or report taking prescription medication, the University Health Center may require you to submit a current treatment plan or other supporting medical documentation from your treating provider. This documentation helps ensure continuity of care and enables the University Health Center to appropriately support your healthcare needs while you are enrolled at Morgan State University.

Treatment plan documentation must:

• Be prepared on the healthcare provider's official letterhead.
• Include the healthcare provider's signature.
• Include the healthcare provider's office stamp (if applicable).
• Describe your current diagnosis or medical condition, as appropriate.
• List all current prescription medications, including dosage and frequency, if applicable.
• Outline your current treatment plan and any recommendations for ongoing care or follow-up.
• Include any activity restrictions, accommodations, emergency care instructions, or other information the University Health Center should be aware of, when applicable.

Examples of treatment plan documentation may include but is not limited to:

• A provider letter summarizing your current medical condition and treatment.
• A medication management plan.
• An asthma action plan.
• A diabetes management plan.
• A seizure action plan.
• A severe allergy or anaphylaxis action plan.
• A mental health treatment plan or provider statement, when applicable.

Documentation requirements vary based on your individual health history. If additional information is needed, the University Health Center will notify you directly through your MSU email account. 

Note: Please answer all health history questions completely and accurately. Incomplete questionnaires or missing required documentation may delay your medical clearance.

Review and electronically complete all required forms before submitting your medical records.

These may include:

• Consent to Treat
• Notice of Privacy Practices
• Patient Rights and Responsibilities
• Financial Responsibility Agreement
• Emergency Contact Verification
• Medical or Religious Vaccine Exemption Request (if applicable)
  • To locate and submit the Vaccine Exemption Form, please follow these instructions:
    • Click Optional Forms (located on the left side)
    • Click Preview under the Vaccine Exemption Request Form
    • Complete the form, ensure all fields are completed with the all signatures required and submit as a PDF for review.

Students requesting an exemption should upload all required supporting documentation through the Student Health Portal.

Medical clearances require both entering your immunization dates and uploading official documentation.

Enter every vaccine exactly as it appears on your official immunization record.

Verify that each date is accurate before submitting.

Upload Your Official Immunization Records

Upload a complete copy of your official immunization record.

Acceptable documentation includes records from:

• Physician offices
• Health departments
• Previous schools or colleges
• Military records
• Official immunization registries

Documentation must:

• Clearly display your full name
• Include your date of birth
• Show all required vaccination dates
• Be legible and complete
• Be uploaded as a PDF whenever possible

Screenshots, incomplete records, or illegible documentation may delay approval.

Students must meet the University's immunization requirements before medical clearance can be approved.

NOTE: Official immunization records must be submitted through the Student Health Portal. Documentation must be legible, include your full name and date of birth, and be issued by a licensed healthcare provider, health department, school, military, or other official immunization registry. 

The University Health Center will review your submitted documentation.

If additional information or corrections are needed, you will receive a secure message through the Student Health Portal.

Please continue monitoring your portal until your medical clearance status indicates that all requirements have been approved.

Submitting documentation does not automatically satisfy your medical clearance requirements.
