Student COVID-19 Vaccine Information

To meet the vaccination requirement, all students will be required to confirm their vaccination status within the Morgan State Student Health Portal Individuals must have their vaccination document available on the device from which they are uploading information. 

How to upload your COVID-19 Vaccination Card

  • Log into using your MSU email address and password.
  • Confirm you identity by inputting your date of birth.
  • Select Immunization Upload located in the Main Menu.
  • Select Add Immunization Record... to upload an image of your Vaccination Document (files accepted include: PNG,JPG,JPEG, or GIF).
  • Click the SAVE button.

NOTE: Please be advised, if you experience an "error message" while attempting to log into , please send an email to with your Full Name, Date of Birth, Student Identification Number, and attach your COVID-19 vaccination documentation. 

How to request a COVID-19 Vaccination Exemption

You may request a vaccine exemption if you have an allergy to the COVID-19 vaccine or an underlying medical condition that prevents you from receiving the COVID-19 vaccine. If seeking a medical exemption, you must consult with your physician and complete the Medical Exemption Form.

Medical Exemption Form

  • Log into using your MSU email address and password.
  • Confirm you identity by inputting your date of birth.
  • Select Immunization Upload located in the Main Menu.
  • Select Add Immunization Record... to upload an image of your Medical Exemption Form (files accepted include: PNG,JPG,JPEG, or GIF).
  • Click the SAVE button.

Please allow a minimum of 10 business days to review your Medical Exemption Form once recieved. 

Remote Telehealth Services During the COVID-19 Pandemic:

During the COVID-19 pandemic, the University Health Center staff will offer telehealth services to assist and support students during the remote learning period. If you would like to schedule a video telehealth appointment with a Provider between the hours of 9:00 a.m. - 5:00 p.m. please go to

  • Diagnosis and treatment of acute illnesses and injuries
  • Treatment and monitoring of chronic illnesses under the direction of your physician
  • Immunizations including MMR, Tetanus, Meningitis, hepatitis A & B, seasonal flu, and others
  • Tuberculosis Routine

  • Pharmacy services for prescriptions written by UHC providers
