Change of Academic Catalog Petition

Purpose: This form is to be used if a student wishes to change the academic catalog they matriculated under.

Change or Declaration of Major/Minor Form

Purpose: This form is used if a student wishes to change or update their major, minor, or concentration in the university's student information system.
Procedure: Click on the link below to begin completing the form. This form requires the signature of the chairperson for each major/minor. Please click here for a list of active chairpersons. You will receive an email notification from records@morgan.edu once processed.

Duplicate Diploma Request

Purpose: To request a duplicate copy of your university diploma.
Procedure: Please use the link below to order through Parchment. Once logged in click "Send to yourself, another individual, or third-party." A menu option will appear to select Duplicate Diploma.
Duplicate diplomas are ordered once a month through our 3rd party vendor. Once received, we will mail or hold your diploma for 90 days depending on your selection. Diplomas held for more than 90 days will be destroyed and a duplicate diploma must be ordered.
            

** Faxes and email requests are not permitted as there is a fee associated with this process.**

Enrollment Verification

Purpose: To request a letter from the University verifying enrollment or degree completion for a student.
Procedure: Click on the link below to begin completing the form. You must use your Morgan State email address. You must also attach your Valid Photo ID. You will receive an email notification from records@morgan.edu once processed.

Exception to Enrollment Policy Appeal

PurposeThis form is used to request an exception to one of the following enrollment policies: last day to cancel enrollment, last day to drop a course(s), last day to drop a course(s) with a grade of "W" and last day for term withdrawal.
Procedure
Complete the online google form and submit to the Office of the Registrar for processing. Exceptions appeals are coordinated by the Office of the Registrar and reviewed by the Exception to Enrollment Policy Committee every two weeks.

**Please be sure to follow all instructions on the form as incomplete appeals will not be reviewed by the Committee.**

Personal Information Update

Purpose: To update personal information such as phone number, address, name, date of birth, and SSN information. (Note: While SSN information is not required in academic records, it is necessary for financial aid eligibility or if you wish to be provided with your student 1098-T form for tax purposes.)

Permission To Take A Course Off-Campus Form

Purpose:  This form is for students who wish to request to take a course at another college or institution and have the credits applied to an equivalent course at Morgan State University.

Procedure: Click on the link below to begin completing the form. Please only submit one course per form. The form requires the signature of your major advisor, chairperson, and dean. The form also requires an attachment of the course description at the requested off-campus school. You must use your Morgan State email account. You will receive an email notification from transfer-evaluation@morgan.edu once processed.

Processing Time: 1-10 Business Days (Due to routing for required signatures, it may take additional time for the form to be received by the Transfer Evaluation Team to process.)

Re-Admission Application

Purpose: To request re-admittance to the university after one year or more of separation. The student applying for readmission must be in good academic standing at all schools attended to be considered for readmission to Morgan State University.
Procedure: Complete the application using the link below. You will receive a decision via email from records@morgan.edu.

*Readmission to the institution is conditional until final official transcript(s) from the previously attended institution(s) are received.*

Residency Reclassification Petition

Purpose: This form is intended for continuing students who seek a change in residency classification. 

Note: Undergraduate and graduate students in their first semester are not eligible to submit this petition; it is intended for continuing students only.


Procedure: Download the form, print, sign, and submit via certified post mail to the Office of the Registrar with ALL supporting documentation. If you require notarization of your documents, please click here to schedule an appointment.

***Incomplete petitions with questions omitted or lacking the required documentation will not be evaluated.  Fax, email, and non-certified mail petitions will not be accepted.***

Summer Session 1 2020 - Petitions will be accepted once the student has registered for courses. The deadline for submitting the petition is June 3, 2020, at 5:00 PM or postmarked by June 3, 2020.

Summer Session 2 2020 - Petitions will be accepted once the student has registered for courses. The deadline for submitting the petition is July 15, 2020, at 5:00 PM or postmarked by July 15, 2020.

Please note that the last day to submit this petition is the last day of late registration in any given semester. Those dates can be found on the Academic Calendar.

Senior Citizen Tuition Waiver

Purpose: This form is intended for use by senior citizens who are registered for courses during a Spring, Summer, or the Fall semester. The student applying must submit: 1) current proof of retirement income  2)Valid government-issued ID as proof of age over 60. The form and documentation must be submitted with the form. (Please note: the waiver cannot be applied retroactively.)

Withdrawal Request Form

Purpose: Students intending to cancel or withdraw from the entire semester or university.

Procedure: Advisement from Academic Retention is strongly suggested. Click on the link below to begin completing the form. You must use your Morgan State email account to complete the form. You must also attach a valid photo ID. You will receive an email notification from records@morgan.edu once all approvers have signed.
