Environmental Health Request Forms
Laboratory safety is essential to protecting the health and well-being of all lab users while ensuring compliance with regulatory standards such as OSHA’s Hazard Communication Standard (29 CFR 1910.1200) and other applicable safety codes. Proper training helps prevent accidents, promotes safe practices, and ensures that everyone working in the lab understands potential hazards and how to manage them responsibly. Please complete the form below to sign up for or request laboratory safety awareness training. You will receive an email confirmation of your registration immediately after submission.
The Laboratory Safety Training Request Form is designed to provide faculty and staff with a clear and formal process to request essential laboratory safety training. By completing this form, individuals ensure that their training needs are communicated to the Office of Safety, Health, and Environment, allowing for the scheduling of appropriate sessions that address laboratory hazards, emergency procedures, personal protective equipment, and compliance requirements. Submitting this request helps maintain a safe and compliant laboratory environment, ensures participants are properly prepared to work in laboratory settings, and supports Morgan State University’s commitment to protecting the health and safety of its community.

Importance of Risk Assessment in MSU Laboratories
At Morgan State University (MSU), maintaining a safe and compliant research environment is a shared responsibility among all laboratory users, supervisors, and Principal Investigators (PIs). Conducting a risk assessment is a critical step in ensuring that every experiment, procedure, or operation is performed safely and in accordance with regulatory standards.
A risk assessment helps identify potential hazards associated with laboratory activities—including chemical, biological, physical, and radiological risks—and determines appropriate control measures to eliminate or minimize these risks before work begins. By evaluating potential exposures and implementing preventive strategies, we can protect our faculty, staff, students, and the environment from accidents or unsafe conditions.
All lab users, supervisors, and PIs are required to complete a Risk Assessment Form for each experiment or laboratory task conducted on campus. The completed form should be submitted to the Office of Safety, Health, and Environment (OSHE) for review and recordkeeping prior to beginning the work.
This process ensures that laboratory operations align with applicable regulatory requirements, including
-
OSHA 29 CFR 1910.1450 – Occupational Exposure to Hazardous Chemicals in Laboratories (Laboratory Standard):
This regulation requires that employers evaluate potential hazards and implement controls to protect laboratory personnel from exposure to hazardous chemicals. It emphasizes the need for a chemical hygiene plan and prior approval for particularly hazardous operations. -
29 CFR 1910.132 – Personal Protective Equipment:
Requires employers to assess the workplace for hazards that necessitate the use of PPE and ensure that appropriate protective equipment is provided and used. -
29 CFR 1910.1200 – Hazard Communication Standard:
Mandates that all hazardous chemicals are properly classified and that employees are informed about their potential hazards through labels, Safety Data Sheets (SDS), and training. Risk assessment supports compliance by ensuring proper hazard communication and control.
Completing a risk assessment not only fulfills compliance obligations but also promotes a culture of safety and accountability within the MSU research community.
Please download and complete the Risk Assessment Form below and submit it to OSHE@morgan.edu for review before beginning your laboratory work.
RISK ASSESSMENT FORM
The purpose of the Chemical Inventory/Hazardous Waste Removal Request Form is to provide a structured process for tracking, managing, and safely disposing of hazardous chemicals on campus. This form ensures that laboratories, research spaces, and other facilities remain in compliance with federal, state, and university regulations, while also reducing risks to health, safety, and the environment. By documenting the type, quantity, and location of chemical waste, the form creates an official record that supports regulatory reporting, hazard communication, and accountability. It also allows the Office of Safety, Health, and Environment (OSHE) to verify that materials are properly labeled, packaged, and prepared for removal in accordance with Environmental Protection Agency (EPA), Occupational Safety and Health Administration (OSHA), and National Fire Protection Association (NFPA) standards.
OSHE collaborates closely with Triumvirate Environmental, the university’s contracted hazardous waste management vendor, to carry out the removal and disposal process. Once a request form is submitted, OSHE reviews it for completeness and accuracy, then coordinates with Triumvirate to schedule pick-ups, provide access to designated storage areas, and ensure all waste is handled in compliance with legal and safety requirements. This partnership ensures a seamless process where OSHE maintains oversight and regulatory compliance, while Triumvirate brings specialized expertise in hazardous waste handling, thereby minimizing risks and supporting a safe and sustainable campus environment.
