Safety Incident Report

The General Suggestion Form provides members of the Morgan State University community with an opportunity to share ideas, recommendations, or concerns that may improve campus operations, safety, and overall quality of services. The form serves as a structured tool to capture input consistently, ensuring all suggestions are documented, reviewed, and considered by the appropriate department.

  • Purpose: Document incidents involving safety hazards. Records relevant details, including the date/timing, issues noted, and corrective actions needed. Allows staff, faculty, or students to report hazards, unsafe practices, or near-misses on campus.

  • Importance of Completion: Provides an official record for rectifying safety concerns on campus with actionable information to prevent accidents before they happen.

General Suggestion Form

The General Suggestion Form provides members of the Morgan State University community with an opportunity to share ideas, recommendations, or concerns that may improve campus operations, safety, and overall quality of services. The form serves as a structured tool to capture input consistently, ensuring all suggestions are documented, reviewed, and considered by the appropriate department.

  • Purpose: Provide a structured way for faculty, staff, and students to share safety, health, and environmental ideas. Capture recommendations for training needs, awareness campaigns, equipment upgrades, or facility improvements. Encourage proactive participation in strengthening Morgan State University’s safety culture. Support OSHE in identifying opportunities for continuous improvement across campus operations.

  • Importance of Completion: Ensures OSHE receives clear and detailed information to evaluate suggestions effectively. Allows the office to assess feasibility, benefits, and alignment with compliance requirements. Helps prioritize recommendations that can positively impact safety and health outcomes. General Suggestion Form

Compliance Training Request

A compliance training request is a formal submission used by departments, supervisors, or individual employees to ask OSHE to provide training that addresses regulatory, institutional, or safety-related requirements. It ensures that employees receive the necessary knowledge and skills to comply with OSHA, NFPA, EPA, and university-specific policies.

  • Purpose: Used to request mandatory safety trainings such as Laboratory Safety, Fire Safety, Defensive Driving, Hazard Communication, First Aid/CPR/AED, etc. *

  • Importance of Completion: Ensures employees are enrolled in the right session, allows OSHE to track compliance with training requirements, and provides documentation that employees have met OSHA and NFPA standards. Incomplete forms may delay training access or create compliance gaps.*

Hot Work Permit

The OSHE department shall inspect the area before approval is granted for all hot work permits as required by the code. Failure to submit a hot work permit shall result in the immediate issue of a cease and desist citation. Fire watch shall be identified, and the fire extinguishing element shall be inspected and shall remain on the work site for the entire duration while work is ongoing.

Risk Assessment Form

To proactively mitigate workplace risks, the Office of Safety, Health, and Environment (OSHE) has implemented comprehensive risk assessment forms. These forms are designed to guide supervisors and managers through a structured process of identifying, evaluating, and controlling hazards associated with high-risk tasks and training activities.
Supervisors and managers are required to:

  • Review the planned task or training event to identify all potential hazards, including but not limited to physical, chemical, biological, and ergonomic risks.
  • Assess the likelihood and severity of each identified risk, taking into account the specific work environment, tools, materials, and personnel involved.
  • Determine appropriate mitigation strategies for each risk, such as engineering controls, administrative procedures, use of personal protective equipment (PPE), and staff training.
  • Document all findings and mitigation measures in the risk assessment form, providing sufficient detail to demonstrate due diligence and compliance with OSHE policies.
  • Submit the completed risk assessment form to the OSHE department for review and approval before initiating any high-hazard task or training.
  • Communicate the approved risk mitigation measures to all personnel involved in the task or training, ensuring everyone understands their responsibilities and the required safety precautions.
  • Monitor the task or training activity, making adjustments to controls as necessary and documenting any changes or incidents that occur.

Timely completion and submission of the risk assessment forms are essential for maintaining a safe workplace, preventing injuries, and minimizing the risk of property loss. Failure to comply with these requirements may result in disciplinary action and increased scrutiny of departmental safety practices.
