Undergraduate Psychology Forms

Research Track Advising Form

  • Be sure to spell your advisor's name and email correctly.
  • On the form itself, you will need to enter your name and MSU ID number.
  • You will need to attach a PDF of your Degreeworks course records. Your records must show your complete course and degree progress.
  • Mark each course you want to take on the form. Leave all other courses blank.

Override Form

  • Complete all fields, including the name and email of your academic advisor.
  • Upload a PDF of your DegreeWorks course records in the appropriate space on the form. Your records must show your complete course and degree progress.
  • If you request an override for one of the following reasons, you must also upload the corresponding documentation:
    • Academic Recovery - Requires documentation from Continuing Studies office
    • Job Conflict - Requires a letter from your employer
    • Graduating Senior - Requires documentation from your department
    • Financial hold - Requires documentation from Websis or the Office of Financial Aid
  • If approved, you will receive an email from psychology@morgan.edu with confirmation. After you receive this email, you may register for the course using the CRN.

Grade Appeal Form

  • This form initiates a grade appeal for a psychology course.
  • Be sure to follow all directions carefully and attach all required documentation.

Change or Declaration of Major/Minor/Concentration/Track Form

NOTE: Change of Major/Minor/Concentration/Track requests have changed. Students must sign into the Student Self-Service/Websis to initiate and submit this form.

Complete this request to change or update your major, minor, track, or concentration in the university's student information system.

  • Complete all fields, including the chairperson of your current major and the chairperson of your new major. The names of the chairpersons can be found on the programs' webpages if you are unsure.
  • Spell all names and email addresses correctly.
  • You will receive an email notification from records@morgan.edu once the form has been processed.
