VES Online Forms

Please read the form explanations below to ensure that you have gathered all necessary supporting documentation to complete the forms prior to starting the form. Documents are not considered submitted until you select "SUBMIT or FINISH" at the end of each page.

NOTE: Documents completed via DocuSign cannot be left idle for more than 15 minutes, or the windows will automatically close. Use a computer or laptop device when completing forms for the best outcomes and submissions. All forms are submitted into a queue and processed in the order they are received. Completion deadlines per process are provided within each document's descriptions below. 

Military Funding Request Form  

*Complete ONLY one (1) form per term - USE COMPUTERS/LAPTOPS ONLY!!

Summer Session I 2023 (5/31 -6/28)                         Summer Session II 2023 (7/5 -8/2)

Summer Full Term 2023 (5/31 - 8/2)                         Fall 2023 (TBA)

This form is for any student who holds military-affiliation status. (ROLL CALL: All Active Duty members, Select Reservists, National Guard members, Air Guard, Coast Guard, Marine, Retired Veteran, Dependent, Spouse, or ROTC Cadet. You are required to complete this form each term that the student is enrolled in courses at Morgan State.

PROCESSING TIME: 3 - 30 business days***

Enrollment Information Update Form

This form is to be completed by any military-affiliated student who has already completed the Military Funding Request Form for processing. This form will allow you to update/change or revise any previously submitted information.  (i.e. add/drop classes, academic major changes, overrides, address change, withdrawals, incompletes, etc.). This form can be completed multiple times, based on the number of changes that may occur with your student profile over the course of any given term.

Retroactive Military Funding Request Form

This form is for any student who has recently been approved for VA Educational Benefits under the Department of Veterans Affairs and requires a previously attended term to be retroactively processed.  As per VA policy, schools can submit retroactive terms up to 1 year to date. For more information on retroactive processing additional instructions are provided on the form.

Yellow Ribbon Interest Form

This form is to be completed by a Yellow Ribbon Program-eligible student who is requesting consideration for the next upcoming major term. (FALL and Spring ONLY)

Off-Campus VA Educational Benefits Certification Request

This form is to be completed by any student veteran who is approved to take a class(es) off-campus and intends to utilize their VA Educational Benefits at a "guest" institution for the next upcoming term.

VES Hold Removal Form

This form is to be completed by a student veteran who is requesting their VA Hold to be released temporarily.

NOTE: All released HOLDs will be re-applied at 12:01 am the following day. All HOLDs will be automatically released prior to the next term registration opening.

PROCESSING TIME: 1 - 3 Business days* (preferred 5-7 Business day advance notice)

*No same-day responses

VES SAP Compliance Form (Opens Mid-term period ONLY)

This form is to be completed by any student veteran who is not meeting Satisfactory Academic Progress (SAP) for VA Compliance for any given term.

SUBMISSION DEADLINE: 30 business days before the end of the term. (Spring/Fall)*

*Non-standard terms 7-10 business days before the end of the term.

Military Leave Notification Form

This form is to be completed by any student veteran who is being called to military duty during the course of a current term in which the student is enrolled. This applies to student veterans called to military service, military training, or emergency military leave. (See Military Leave Policy)

PROCESSING TIME: 10 - 15 Business days before departure

VES Summer 2023 Tuition Assistance Scholarship Application (Opening late Spring)

This application is open to any military-affiliated student, using Chapter 30, Chapter 1606, Chapter 35, Chapter 33 (50%-90% entitlement ONLY), MD NG State Tuition Waiver, Federal Tuition Assistance, ROTC Bridge, ROTC Grant, Exhausted benefits or Self-Pay. Tuition Assistance scholarships are designed to bridge the gap between the student's unmet financial need and the billing settlement. The maximum scholarship award is $500 per student applicant. Awards amount decisions are based on the students' express needs, current account status, and required document submissions. For additional scholarship information and requirements CLICK HERE!

VES Summer 2023 Book Scholarship Application (Opening late Spring) 

This application is open to any military-affiliated student who is using Chapter 30, Chapter 1606, Chapter 35, MD NG State Tuition Waiver, Federal Tuition Assistance, ROTC Bridge, ROTC Grant, Exhausted benefits, or Self-Pay. Eligible students Must have a GPA of 2.5 or higher, provide documented proof of book costs, and MUST participate in the Morgan State University First Day Complete Bookstore Program. Book scholarship awards range up to $400 per student account based on additional requirements and account status. For Graduate student requirements and additional scholarship information CLICK HERE!
