Medical Clearances

Overview

All undergraduate and graduate students—whether attending in-person or virtually—are required to complete their health requirements. This includes:

• Updating your student health profile
• Completing your health history
• Completing required forms and consents
• Submitting immunization records

Important:
Failure to complete these steps will result in a registration hold, preventing you from enrolling in classes.

Important Dates & Deadlines

• CASA Academy Students: The Student Health Portal opens May 11, 2026, and medical Clearances must be completed and submitted by June 5, 2026.
• General Student Deadline:
  All students (undergraduate and graduate—whether attending in person or virtually) who are not participating in BEAR (NSO) or CASA Academy must complete all health requirements by July 31, 2026.
• BEAR (NSO) Students: The Student Health Portal opens April 1, 2026, and medical Clearances must be completed and submitted by the date listed below, based on your scheduled Orientation date.

Step 1: Log in to the Student Health Portal

• Access the Student Health Portal via the University Health Center website
• Log in using your Morgan credentials
• All requirements must be completed within this system

Step 2: Update Your Profile (Start Here First)

Review and confirm the following:

• MSU Students Phone number
• Morgan State University email address
• Current address (on-campus or off-campus)
• Emergency contact name and phone number

Important:
You will not be able to move forward until Step 2 is completed.

Step 3: Complete Your Health History

Complete all sections of your health history, including:

• Allergies
• Medications
• Medical Conditions
• Family History
• Hospitalizations
• Surgeries/Procedures

Step 4: Complete Required Forms and Consents

Required Consent Forms

• Consent to Treatment
• Consent To Treatment(minor), only a guardian can sign for the student
• Notice of Privacy Practices
• Financial Policy
• Patient Rights

Optional Form (If Applicable)

• Vaccine Exemption Request

Instructions for Exemption Form:

• Click Optional Forms
• Click “Preview” under the Vaccine Exemption Request Form
• The Student Vaccination Exemption Form will appear for completion

Step 5: Submit Immunization Records

You must complete both steps below:

1. Enter Immunization Dates

• Enter all vaccine dates exactly as shown on your records

2. Upload Immunization Document

• Upload a complete, official immunization record
• Do not upload partial or incomplete documents; we only accept the full PDF that includes your full name and date of birth

Required Immunizations

MMR (Measles, Mumps, Rubella)

• 2 doses (after 12 months of age, spaced at least 28 days apart)

Tdap (Tetanus, Diphtheria, Pertussis)

• 1 dose within the last 10 years

Meningococcal (Meningitis)

• 1 dose administered at age 16 or older

Hepatitis B

• 2- dose series (0 and 1 month)
• 3-dose series (0, 1, and 6 months)

Varicella (Chickenpox)
Provide one of the following:

• 2 vaccine doses
• Lab proof of immunity (titer)

Tuberculosis (TB) Screening (if required)
Required for:

• International students
• Students identified as higher risk

Sickle Cell Screening (Student-Athletes Only)

• Required for ALL student-athletes only

Submission Requirements

• Upload the full immunization record. We only accept the full PDF that includes your full name and date of birth.
• Enter all vaccine dates accurately based on the information listed on your Immunization records.
• Ensure records are official and legible when scanned into the system via PDF.

Step 6: Monitor Your Status

• Check the portal regularly for updates
• Complete any requested corrections promptly
