The University Health Center welcomes all students attending the CASA Academy at Morgan State University. During CASA Academy, future Morganites will gain a wealth of knowledge from our University Health Center staff. All CASA Academy students are required to submit their Health Entrance Certificate and full Immunization Records (including (bivalent) COVID-19 vaccination information indicating full vaccination (including booster dose) no later than Friday June 21, 2024.
NOTE: According to the Center for Disease Control and Prevention, on May 11, 2023, the Public Health Emergency Declaration for COVID-19 (Coronavirus) expired. With this updated information and guidance from the Morgan State University Health, Safety and Wellness Committee, we've updated our guidelines regarding COVID-19 vaccination. COVID-19 vaccination remains highly recommended but is no longer required for Morgan State University students. Additionally, we recommend all Residential and Immuno-compromised students be up to date with the COVID-19 vaccine and booster doses.

How To Submit Your Health Entrance Certificate:
- Complete & Sign your Health Entrance Certificate. Please complete and sign the Health Entrance Certificate.
- Submit your Health Entrance Certificate. Please email a copy of your Health Entrance Certificate. to healthcenter@morgan.edu. Please include your full name, student identification number, and date of birth in the email. Additionally, please include CASA Academy in the subject line of the email.
- Submission Deadline. The submission of your Health Entrance Certificate is Friday, June 21, 2024 for all CASA Academy students.
NOTE: Please complete ALL sections of the Health Entrance Certificate. If you fail to complete ALL sections, we will return the document. We will request a Treatment Plan from your Physician for all sections marked YES in the Personal History Section of the Health Entrance Certificate.
The University Health Center staff reviews all documents submitted for verification. Please allow 1-3 business days for the reviewal of documents and updating your student chart. Morgan State University Student Health Center requires proof of the following Immunizations and TB Test:
- Immunization for MMR #1 and MMR #2 (Measles, Mumps, Rubella)
- Hepatitis B (1,2,3)
- Tdap (Diphtheria, Tetanus, and A cellular pertussis booster vaccine within 10 years)
- Meningitis vaccine
- Tuberculosis Test (TB/PPD)- ONLY REQUIRED FOR INTERNATIONAL STUDENTS. Test must be administered within the past calendar year
The Immunization Records are mandatory and must be documented with dates administered by the health care provider or lab.
Morgan State University Student Health Center recommends but does not require proof of the following Immunizations:

How To Submit Your Immunization Records and TB Test:
- Collect your documents. Please collect a copy of your Immunization Records and TB Test Results.
- Submit your documents. Please email a copy of your Immunization Records and TB Test Results to healthcenter@morgan.edu . Please include your full name, student identification number, and date of birth in the email. Additionally, please include CASA Academy in the subject line of the email.
