How To Submit Your Health Entrance Certificate:

1. Complete & Sign your Health Entrance Certificate. Please complete and sign the Health Entrance Certificate.
2. Submit your Health Entrance Certificate. Please email a copy of your Health Entrance Certificate. to healthcenter@morgan.edu. Please include your full name, student identification number, and date of birth in the email. 
3. Submission Deadline. The submission of your Health Entrance Certificate is Friday, June 2, 2023 for all Bear Necessities New Student Orientation students.

NOTE: Please complete ALL sections of the Health Entrance Certificate. If you fail to complete ALL sections, we will return the document. We will request a Treatment Plan from your Physician for all sections marked YES in the Personal History Section of the Health Entrance Certificate.

The University Health Center staff reviews all documents submitted for verification. Please allow 1-3 business days for the reviewal of documents and updating your student chart. All incoming Morgan State University Undergraduate and Graduate students are required to submit their Immunization Records prior to attending the University. The submission of your Immunization Requirements and TB Test must be submitted prior to the deadline below or your Morgan State University registration will be denied. 

Morgan State University Student Health Center requires proof of the following Immunizations and TB Test:

• Immunization for MMR #1 and MMR #2 (Measles, Mumps, Rubella)
• Hepatitis B (1,2,3)
• Tdap (Diphtheria, Tetanus, and A cellular pertussis booster vaccine within 10 years)
• Meningitis vaccine
• Tuberculosis Test (TB/PPD)- ONLY REQUIRED FOR INTERNATIONAL STUDENTS. Test must be administered within the past year

The Immunization Records are mandatory and must be documented with dates administered by the health care provider or lab. 

Morgan State University Student Health Center recommends but does not require proof of the following Immunizations:

• COVID-19 vaccine

Additionally, All Student Athletes are required to submit a Sickle Cell Test with their Immunization Records. If you would like to receive a Sickle Cell Test at the University Health Center, please follow the instructions on the Services and Fees web page, to ensure you pay for the Sickle Cell Test, before contacting the University Health Center.

How To Submit Your Immunization Records:

1. Collect your Immunization Records. Please collect a copy of your Immunization Records and TB Test Results.
2. Submit your Immunization Records. Please email a copy of your Immunization Records to healthcenter@morgan.edu. Please include your full name, student identification number, and date of birth in the email. 
3. Submission Deadline.The submission of your Immunization Records is due by August 1st for Fall Enrollment and January 1st for Spring Enrollment.

The University Health Center staff reviews all documents submitted for verification. Please allow 2-3 business days for the Health Center staff to review your documents and update your chart. The Morgan State University Health Center requires all students to submit a copy of their Health Insurance Card to support the cost of processing labs and additional fees for services during their appointment at the University Health Center. THIS IS NOT THE STUDENT HEALTH INSURANCE WAIVER.

All students must upload their health insurance card to the MSU Health Portal by August 1st for Fall semester enrollment and January 1st for Spring semester enrollment. 

1. Take A Picture or Scan the front and back of your insurance card. Please ensure your Health Insurance is active. Additionally, please ensure both images are clear. 
2. Log into the MSU Health Portal using your MSU login credentials. You will be required to confirm your date of birth.
3. Select Insurance Card on the left side of  and click on Add New Card. Once you have successfully added the photos of your Insurance Card and the Insurance Information, please verify all information submitted is accurate without typographical errors. Click Save.
