The University will review requests for medical and religious exemptions to the vaccination mandate, as provided for under Title VII of the Civil Rights Act of 1964 and the Americans with Disabilities Act.

Exemption requests can take up to 10 business days to review once received.

MEDICAL EXEMPTION

You may request a vaccine exemption if you have a condition the Centers for Disease Control and Prevention (CDC) considers a contraindication for the COVID-19 vaccination.  Contraindications include a severe allergic reaction (e.g., anaphylaxis) after a previous dose or to a component of the COVID-19 vaccine or an immediate allergic reaction of any severity to a previous dose or a known (diagnosed) allergy to any component of the vaccine. View additional details on contraindications.

Please keep in mind that the CDC recommends that individuals with most pre-existing conditions receive the vaccine to protect their health and safety. View more information from the CDC.

If seeking a medical exemption, you must consult with your physician and complete the Medical Exemption Form.

The medical exemption submission portal is different for students and employees. The instructions are described below.
