Vaccine Exemption

The University will review requests for medical and religious exemptions to the vaccination mandate, as provided for under Title VII of the Civil Rights Act of 1964 and the Americans with Disabilities Act.

Exemption requests can take up to 10 business days to review once received.

MEDICAL EXEMPTION

You may request a vaccine exemption if you have a condition the Centers for Disease Control and Prevention (CDC) considers a contraindication for the COVID-19 vaccination.  Contraindications include a severe allergic reaction (e.g., anaphylaxis) after a previous dose or to a component of the COVID-19 vaccine or an immediate allergic reaction of any severity to a previous dose or a known (diagnosed) allergy to any component of the vaccine. Please keep in mind that the CDC recommends that individuals with most pre-existing conditions receive the vaccine to protect their health and safety. View more information from the CDC.

If seeking a medical exemption, you must consult with your physician and complete the Medical Exemption Form.

The medical exemption submission portal is different for students and employees. The instructions are described below.

STUDENTS: Submit the completed form within the Morgan State Student Health Portal: https://myhealthportal.morgan.edu/. Individuals must have their completed form available on the device from which they are uploading the information.

  • Log-in to the portal using your MSU email address and password
  • Confirm your identify by inputting your date of birth
  • Select “Immunization Upload” from the Main Menu
  • Select “Add immunization record…” to upload an image of your vaccination document (files accepted include: PNG, JPG, JPEG, or GIF)
  • Click the “Save” button

EMPLOYEES: Submit the completed form here: MSU Medical Exemptions. Individuals must have their completed form available on the device from which they are uploading the information.

  • Enter your MSU email address, First Name, Last Name, and Company. Click the “Continue” button.
  • Drag the file from the device location to the webpage or click “Browse files” to locate the file on your device.
  • Click the “Upload” button.

RELIGIOUS EXEMPTION

You may request a vaccine exemption if sincerely held religious beliefs or practices conflict with the COVID-19 vaccination requirement.  If seeking a religious exemption, you must complete the Religious Exemption Form.

  • Click the “Upload” button.
