NEW STUDENTS

 

Dear New Student,

I am happy to welcome you to Morgan State University and hope you will find your experience here challenging, enriching and fun. As an adult, you are responsible for choices concerning your health, but we are here to provide important options, information, resources and medical care. Your cooperation is essential. Please complete the following checklist before mailing your medical record to:

____1. Complete the enclosed health history form and return documents to University Health Center with all required immunization records.

____2. Secure copies of official immunization records from your current health care provider. Acceptable proof of immunization includes: a copy of your high school immunization record, or a copy of the original immunization certificate.(Note: Only proof of vaccination of immunity by blood titer is acceptable. History of the disease is not sufficient evidence of immunity.).

Required Immunization/Screening Include:
__ 2 doses of Measles vaccine administered after the first birthday
__ 2 doses of Rubella administered after the first birthday
__ 2 doses of Mumps administered after the first birthday
__ Tetanus/Diphtheria 3 doses of DPT or DT
__ Tetanus/Diphtheria booster within 10 years
__ Tuberculosis skin test results within 6 months (even if history of BCG). If contraindicated, or positive results, a chest x-ray is required

____3. Have your health care provider complete the Health Entrance Certificate. If you are chronically ill, regularly take medications or allergy injections, or will be coming to Morgan State University with an unhealed injury, please submit to the Health Center a complete report from your Health Care provider including plans and/or restrictions.

Sports clearance for varsity or club sports requires records concerning any recent orthopedic or neurological problems, as well as any cardiac problems.

____4. Tuberculin test done at physical exam must be read within 49-72 hours and documented by health care provider on the physical exam form. Forms will not be acceptable without your TB test results.

____5. Before mailing medical records, check to be sure you have all of the above. Please retain a copy of all records for your personal files.

Morgan State University does not discriminate against any person with any health problems. You have already been accepted, and none of the information called for on the enclosed forms can jeopardize your admission. Your medical records are held in strict confidence at the Health Center, and they will not be shared with anyone without your permission. It is in your best interest that these records be complete.
