2012 Health Benefits Open Enrollment
Health Benefits Open Enrollment (April 17, 2012 through May 1, 2012)
Regular and Contractual Employees will be able to enroll in health benefits during this period. Regular Employees will have to use the 24/7 Interactive Voice Response Telephone System to make changes and the Contractual employees should use the enrollment forms that were mailed to their home address.]
Current benefits will rollover into the new fiscal year commencing July 1, 2012, if you are not making any changes. If you are making changes such as an addition, remember to provide documentation (i.e., a marriage certificate, spousal affidavit, birth certificate and the dependent affidavit for each child).
The PIN number for using the (IVR) is your birth date in a four digit format (i.e., April 14th is “0414”), and the telephone number is 410-669-3893 or 1-888-578-6434.
Employees must re-enroll to continue Flexible Spending Accounts each year.
A correction period is scheduled May 9, 2012 through May 16, 2012 for those employees who were unable to enroll during the initial period.
Dependent children are covered up to age 26 for medical benefits and for term life insurance.
Major Changes in PPO and POS Plans
EFFECTIVE JULY 1, 2012 EMPLOYEES PAY A CO-INSURANCE PERCENTAGE PAYMENT FOR “ALLOWED BENEFITS” IN THE PPO AND POS PLANS ONLY.
THE EMPLOYEE MUST PAY 10% OF THE IN-NETWORK ALLOWED BENEFIT COST and 30% FOR OUT OF NETWORK ALLOWED BENEFIT COST.
PLAN YEAR OUT-OF-POCKET MAXIMUMS ARE IN PLACE PER INDIVIDUAL/FAMILY
