HEALTH ENTRANCE CERTIFICATE
Health History is mandatory for all undergraduate students, communter, or residents. This form must be completed & returned to the Student Health Center with all immunizations or your MSU registration will be denied.

MENINGOCOCCAL IMMUNIZATION FORM
Maryland State Law requires every student enrolled at the University and who live in on-campus housing be vaccinated for the meningococcal virus disease, or sign specified waiver.

MENINGOCOCCAL VACCINE WAIVER FORM
Specified waiver that releases the State of Maryland and the University from any liability on account of any loss or personal injury which may result in the non-compliance with the law.

HEALTH INSURANCE WAIVER FORM
If you are covered by health insurance (e.g., through family plans, or employer sponsored plans), you may decline MSU's student health insurance by completing a waiver form. 

HEALTH INSURANCE PURCHASE FORM

Part-time undergraduates and graduate students may purchase health insurance for a fee. See form for details.
